Forms mailed automatically only to:
- Terminated, deceased, or retired individuals
- Covered individuals in CA, NJ, DC, RI
How to Get Your Form:
- Active Employees: can download your Form 1095-C by visiting HuntsmanBenefits.com.
- Former Employees: can request it using one of the following methods:
