Recent changes under the Paperwork Reduction Act (IRS Notice 2025-15) state that Forms 1095-B and 1095-C will no longer be automatically mailed to everyone. Instead, forms will be provided upon request, unless required by state law.

Forms mailed automatically only to:

  • Terminated, deceased, or retired individuals
  • Covered individuals in CA, NJ, DC, RI

How to Get Your Form:

  • Active Employees: can download your Form 1095-C by visiting HuntsmanBenefits.com.
  • Former Employees: can request it using one of the following methods:
